The bust: Lessons learned is a ritual, more accurately called lessons discussed. Two weeks after go-live, the people who made the key calls are already rolling off, and the room rebuilds the story from memory and a few surviving slide decks. What gets written down is the version everyone is comfortable telling. The decision that caused the rework left with the person who made it.
Then the document goes into a folder. The next program starts without reading it, repeats the same mistakes, and this time your name is on the program.
Axiamatic reads requirements, design docs, and meeting recordings as they're produced, preserves every decision and its rationale in a Living Context Graph that analyzes, informs, and recommends, and preempts repeat mistakes in every subsequent program.
Programs don't get better because they held a retrospective. They get better because the lessons are learned implicitly and applied explicitly. Both without any manual effort.
